This form will be available weekdays Monday - Friday from 8:30am until 6:00pm (except bank holidays) and until the request cap is reached.
Enquiry Forms
If you need to contact the Practice about your ongoing care, but do not require an appointment, you can complete one of our online forms using the links below. The forms will normally be available standard working days between 8:00am and 6:00pm Monday – Friday (except bank holidays).
Once your request has been received it will be reviewed by the Practice and passed to the most suitable department. A member of staff will then aim to respond to your request in 1-2 working days..
Our Patient Online Service - SystemOnline
Our computer system gives patients the option to access a range of services online, such as:
- Booking or cancelling appointments
- Ordering repeat prescriptions
- Viewing elements of their medical record.
How to Sign-up
To access this service we will need to set you up with your unique username and password. You can get this from a member of our Reception team at either our Tramways or Middlewood site. We will need to be able to confirm your identity so please bring along a piece of photo ID with you. To maintain patient confidentiality we are unable to issue this password to relatives.
Access for Children Under 16
- Children under 13 are not given their own account. Instead, access is granted through the account of anyone holding parental responsibility.
- Patients aged between 13 and 16 must provide consent to allow continued sharing.
- Upon turning 16, any access granted before will be automatically removed. The patient will need to set up their own account and be responsible for their own username and password.
